Patients with any of the above conditions require regular monitoring and the practice offers appointments with specialist nurses who are able to review medication and conditions. This review is usually due in your birth month but the date can be found at the bottom of your prescription and you will also be reminded by letter. This is a ‘one-stop-shop’ therefore patients with multiple problems will receive a longer appointment but will only usually have to attend once per year for their review with a nurse.

Some patients will require a blood test and urine test before their annual review appointment. The receptionist will arrange this when you call for your review appointment.
